ALW Program Social Work Case Manager
Rebecca joined Senior Care Solutions in 2024 and brings a unique blend of compassion, professionalism, and organizational expertise to her role. Her career reflects a deep and consistent commitment to advocating for underserved communities and connecting individuals with the resources they need to thrive. Rebecca began her career in business, earning an Associate’s degree while working in marketing and business development at a law firm for eight years. It was during this time she discovered her passion for helping others, which inspired her to return to school and pursue a career in social work. She earned her Bachelor’s degree in Social Work from California State University, Sacramento in 2018. During her studies, she interned at a Student Support Center serving three local elementary schools, gaining hands-on experience in case management, family communication, and community referrals. After graduation, Rebecca was promoted to Director of Social Services for several low-income housing communities, where she developed and facilitated resource connections and social programming for residents of all ages. She later expanded her expertise as a case manager at a home health agency through various waiver programs, supporting vulnerable seniors through discharge planning with the goal of keeping them safe at home, home visits, advocacy, and coordination with health insurance providers, government agencies, and financial representatives. Her work emphasized not only care but also empowering clients toward greater self-sufficiency. Rebecca is especially passionate about bridging gaps in care, supporting families through transitions, and ensuring every client feels seen and supported.
“After having the privilege of being my mother’s caregiver during her time on hospice, I gained a deeper understanding of the physical work and emotional stress that a family member goes through. I will never forget the worker who truly listened to my concerns and provided the needed resources during such a difficult season. Their genuine care and compassion lightened the load and I didn’t feel alone. I am grateful to now be able to do the same for other families and give value to the clients I serve by listening to them, asking questions, and being their voice so that they know that they are heard and cared for.”
