Enhanced Care Management (ECM)
Our ECM services are designed to provide comprehensive, personalized support for individuals with complex health needs. Our dedicated team of Lead Care Managers work collaboratively with clients to ensure they receive the necessary care and resources tailored to their unique circumstances.
Key Features of Our ECM Services:
- Personalized Care Plans: We develop individualized care plans that address medical, social, and emotional needs.
- Coordination of Healthcare Services: Our team ensures seamless communication between healthcare providers, family members, and clients to optimize care delivery.
- Closed-Loop Referrals: Under ECM, a Lead Care Manager identifies needs and initiates referrals to quality services. Through our closed-loop referral process, referrals are tracked through completion, confirming service delivery and documenting feedback and results.
- Ongoing Monitoring: We conduct regular follow-ups to assess progress and make necessary adjustments to care plans.
Community Supports (CS):
Community Supports are medically appropriate, cost-effective services authorized by managed care plans to address health-related social needs, including access to safe housing and care solutions. Senior Care Solutions is contracted to provide select Community Supports at this time; for services outside our scope, we will facilitate referrals to high-quality organizations that can provide direct delivery of the requested supports.
