We guide clients through the Assisted Living Waiver authorization process—from placement on the state waitlist to completion of the ALW application.

Ongoing Care Coordination
Our commitment to our clients doesn’t end with placement. We provide ongoing care coordination services, including monthly visits, to monitor client well-being and address any emerging needs. This proactive approach helps reduce hospitalizations and ensures stable housing and care.
Resource and Referral Services
We connect clients with a variety of medical, social, and care-related resources, identifying individual needs and providing referrals to ensure comprehensive support. Our extensive network of community resources allows us to address a wide range of issues, from healthcare to social services.
