Care Management Director

Kate serves as the Care Management Director at Senior Care Solutions, where she oversees care coordination services and leads a team of social workers and lead care managers dedicated to helping older adults and their families navigate complex health care and long-term care systems.

Kate has worked with older adults since 2012 and brings over a decade of experience in senior care across a variety of settings, including skilled nursing facilities, assisted living communities, and community-based care management programs. She began her career as a certified nursing assistant while completing her education, gaining firsthand insight into the daily needs and experiences of older adults in long-term care. This experience shaped her passion for social work and advocacy for seniors and their families.

Prior to joining Senior Care Solutions, Kate served as the Social Services Director at a skilled nursing facility, where she supported residents and families through care planning, complex discharge planning, and major life transitions. Today, she specializes in coordinating care for individuals with complex medical and psychosocial needs and has extensive experience navigating Medi-Cal programs, including the Assisted Living Waiver Program(ALWP), CalAIM Enhanced Care Management and community supports.

Kate is passionate about helping families understand their options and ensuring seniors receive the care and support they deserve while maintaining dignity, autonomy, and quality of life.

She earned her Master of Social Work from the University of Central Florida and her Bachelor of Science in Psychology from Appalachian State University.
